Speer Carbon Company, LLC on or about 1/10/24 and times prior thereto:  Frequent inspections were not performed on hooks to identify deformation or cracks.  Hooks were used at the facility in various areas in the facility as part of the hoist lifting operation.  Hoists include, but is not limited to, Budget Series 6000 Man Guard Air Hoist.  c.) Speer Carbon Company, LLC on or about 1/10/24 and times prior thereto:  Frequent inspections were not performed on hoist chains to identify excessive wear, twist, distorted links, or stretch.  Hoist chains were used at the facility in various areas in the facility as part of the hoist lifting operation.  Hoists include, but is not limited to, Budget Series 6000 Man Guard Air Hoist.
