29 CFR 1910.95(d)(1):When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: At the workplace located at 340 Southwest Boulevard, Kansas City, Kansas 66013, employees working in the basement, motor floor, distributor floor, bin deck among other areas were exposed to noise levels above 85dB. The employer did not implement a monitoring program to measure employee noise exposure and identify employees who are required to be included in a hearing conservation program.

29 CFR 1910.272(j)(2)(ii): The employer did not immediately remove any fugitive grain dust accumulations whenever they exceed 1/8 inch (.32 cm) at priority housekeeping areas, pursuant to the housekeeping program, or did not demonstrate and assure, through the development and implementation of the housekeeping program, that equivalent protection was provided. At the workplace located at 340 Southwest Boulevard, Kansas City, Kansas 66013, employees were exposed to fire and burn hazards. The back area of the scale floor, foot of bucket elevator in the basement and other grain handling areas were not kept clean and free from the accumulation of combustible grain dust.
