On or about January 18, 2024, and at times prior thereto, Instance a. In the Bay 1 pit area, employees stood on a metal grating with an uncovered hole while changing vehicle motor oil. Instance b. In the Bay 2 pit area, employees stood on a bent metal grating while changing vehicle motor oil that created an opening and trip hazard.

The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided. On or about January 18, 2024, and at times prior thereto, in the pit areas of Bays 1, 2 & 3, employees moved between the platforms and steps of the bays by stepping through oil and water accumulation on the floor. Grease Monkey International, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(a)(2), which was contained in OSHA inspection number 1695639 citation number 2, item number 1 and was affirmed as a final order on 1/17/2024, with respect to a workplace located at 96 Portsmouth Ave Exeter, NH 03833 Grease Monkey International, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22 (a)(2), which was contained in OSHA inspection number 1689582, citation number 1, item number 1 and was affirmed as a final order on 11/16/2023, with respect to a workplace located at 4517 N Sheridan Rd Peoria, IL 61614.
