29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: a) On or about January 15, 2024, at the Vacuum Feed Prep Unit, an employee, exposed to inadequate PPE, attempted to cut a damaged section of the steam tracing line that was identified near the FI038 VTB Transmitter without a written hazard assessment of the procedure.
