Employee(s) was standing on the forks of the forklift, elevated more than 6 feet above the ground. Fall protection system was not utilized.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the northwest side of the production floor, on or about January 30, 2024, employee(s) were exposed to amputation, and crushing hazards. The guard on the Profimat 50 molding machine was not affixed to the machine, resulting in an amputation to right index finger of an employee.
