29 CFR 1910.36(d)(1): Exit Routes & Emergency Planning - Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. 300 Locust Street - Fabrication Shop: On or about January 24, 2024, Employer did not protect Employees from accidental injury because exit route doors were unable to open from the inside without keys, tools, or special knowledge. 1) Fabrication Shop - East Side Wall / Front Facade: The primary shop egress door had a dead-bolt locking system which would require special knowledge and/or 2-handed operation during an emergency.

29 CFR 1910.101(b): Compressed Gases - General Requirements: The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6. 300 Locust Street (Fabrication Shop): On or about January 24, 2024, Employer did not protect Employees from accidental injury because compressed gas cylinders were not properly stored in accordance with Compressed Gas Association Pamphlet P-1-1965 as specified in 29 CFR 1910.6. Compressed gas tanks were not secured properly from being knocked over or damaged. 1) Fabrication Shop - North Side Cutting Area: A 120cf (approx.) cylinder of inert compressed gas was secured to the rail, of a portable welding curtain that did not secure the compressed gas cylinder in any form.

29 CFR 1910.253(a)(3): Welding, Cutting, And Brazing: Apparatus such as torches, regulators or pressure-reducing valves, acetylene generators, and manifolds used in an oxygen-fuel gas system were not approved. 300 Locust Street - Fabrication Shop: On or about January 24, 2024, Employer did not protect Employees from accidental injury because compressed gas cylinders were not fitted with approved gauges and/or regulators because they were damaged and/or missing their protective lenses making verification of proper and accurate working pressure difficult. Many gauges utilized by the Employer, showed signs of significant damage that could potentially lead to a dangerous flammable gas leak and/or combustion. 1) Fabrication Shop - South Side: Cutting Torch Set 1: Oxygen tank gauges damaged and/or missing calibration. 2) Fabrication Shop - South Side: Cutting Torch Set 2: Oxygen tank gauges damaged and/or missing calibration. 3) Fabrication Shop - South Side: Cutting Torch Set 2: Acetylene-Propane tank gauges damaged and/or missing calibration. 4) Fabrication Shop - North Side: I-Beam Gas Tank Storage Point: Inert gas tank gauges damaged and/or missing calibration.
