29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) 9239 Uravan Ct, Commerce City, CO: On or about January 22, 2024, and at times prior, employees were exposed to fall hazards of approximately 25 feet to the ground below while installing asphalt shingles on the second story of a residential roof. The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) 9239 Uravan Ct, Commerce City, CO: On or about January 22, 2024, employees were exposed to fall hazards of approximately 25 feet to the ground below. The employer did not ensure that extension ladders had side rails extending 3 feet above the top balcony rail or were secured when accessing second level balcony.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m), or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) 9239 Uravan Ct, Commerce City, CO: On or about January 22, 2024, and at times prior, employees were exposed to falls hazards of approximately 25 feet while removing and installing shingles on a two-story residential home without utilizing fall protection.  Crisolito Roofing LLC. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1572636, citation number 1, item 1 and was affirmed as a final order on March 1, 2022, with respect to a workplace located at 17640 Delta Ave in Erie Colorado 80514.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) 9239 Uravan Ct, Commerce City Colorado: On or about January 18, 2024, and at times prior, the employer failed to report a work-related hospitalization within 24 hours of the injury occurring. The event occurred on January 18, 2024 at approximately 10:20 AM and OSHA was not notified until January 19, 2024 at 12:00PM.
