29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about January 23, 2024, employees engaged in waste water treatment activities at the Cargill Meats Solutions facility located at 1529 23rd St, Columbus, Nebraska, were exposed to electrical shock hazards in that the employer was using a portable power tap (power strip) in conjunction with temporary flexible wiring (extension cord) to power two metering pumps. The portable power tap is a piece of electrical equipment designed for use in an office environment for distribution of power to multiple computer type peripherals and not for industrial setting of a meat processing facility in which environmental conditions such as dust and water compromise its safe operability.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating. On or about January 23, 2024, employees engaged in beef processing activities at the Cargill Value Added Meats facility located at 1529 23rd St, Columbus, Nebraska, were exposed to electrical shock hazards in that the exterior insulation and protective covering for the internal wiring on the mobile meat auger had pulled out of its conduit exposing the internal wiring to wet environmental conditions as well as unintentional contact with cleaning tools and equipment that the insulation for that wiring was not designed to protect against.
