29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  a) On or about January 25, 2024, as the controlling employer, McCauley Constructors Inc, did not ensure that subcontractor employees were protected from cave-in hazards while working in a trench without adequate protective systems in place. Subcontractor employees were working in a trench hat had 10 foot near-vertical walls, signs of sloughing, and undercutting where the pipe was being installed. This condition exposed subcontractor employees to cave-in hazards.
