29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. a) On or about January 26, 2024, the employer did not ensure to obtain the manufacturer's approval to use a fabricated "blue work platform" attachment with the (1) TOYOTA Forklift Truck (model - 5FGC30), (2) Yale Lift Truck (model - GLC080VXNGAE094), and (1) MITSUBISHI Forklift Truck (unknown model). Employees are exposed to a fall of approximately 4-5 feet and crushing hazards while using the fabricated "blue work platform" to perform minor servicing to the mill in the production area.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) On or about January 26, 2024, the employer did not ensure the forklift data plate for the (1) TOYOTA Forklift Truck (model - 5FGC30), (2) Yale Lift Truck (model - GLC080VXNGAE094), and (1) MITSUBISHI Forklift Truck (unknown model) was updated to show the identity and approximate weight of the truck and attachment combination prior to using the fabricated "blue work platform". Employees are exposed to a fall of approximately 4-5 feet and crushing hazards while using the fabricated "blue work platform" to perform minor servicing to the mill in the production area.
