29 CFR  1910.212(b): Anchoring fixed machinery. Machine designed for a fixed location was not securely anchored to prevent walking or moving:  A)     In the maintenance shop, there was a bench mounted drill press that was not secured to the top of the work bench to prevent it from moving or tipping over.  The motor of the drill was at the top of the press, giving it a high center of gravity.  Employees using the drill press could be struck by the equipment if it fell over during use.

29 CFR  1910.213(c)(1): Each circular hand-fed ripsaw was not guarded by a hood that completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:   A)     In the maintenance shop, there was a table saw that did not have a hood guard in place above the blade.  Employees operating the saw were exposed to the portion of the blade that was above the stock being used.

29 CFR  1910.215(b)(9): The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch:  A)     In the maintenance shop, there was a bench grinder with its tongue guards installed backwards, so that the guard could not be adjusted to one quarter inch from the surface of the grinding wheel, leaving a gap between the surface of the wheel and the outer casing that was over one-half inch long.  Any portions of the wheel or material being ground would not be stopped from flying out and hitting the employee operating the equipment.

Unmounted fire extinguishers are susceptible to damage.  Damaged fire extinguishers may not work correctly when needed.
