29 CFR  1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  a.  On or about January 30, 2024, in the freezer, the employer did not ensure all exits routes were maintained to allow employees free and unobstructed access to exits.  There were three exit doors blocked with pallets and boxes, thereby exposing employees to fire and smoke hazards.

29 CFR  1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  a.  On or about January 30, 2024, in the freezer, employees were exposed to environmental hazards when the temperature was maintained at approximately negative 10 degrees Fahrenheit (-10 ?).  Employees were exposed to hazards associated with extreme cold when the employer failed to provide head and neck protection to employees required to work in the dangerous environmental condition.

On or about January 30, 2024, in the facility, the�employer had�not conducted�a personal�protective equipment�assessment, which�included the�identity of�the workplace�evaluated, the�person who�conducted the�evaluation and�the date�of the�hazard assessment.��Employee exposure�included, but�was not�limited to, exposure to temperatures below zero and struck-by hazards.
