OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a) New commercial construction located at 763 Highway 96, Bonaire, GA 31005. On or about 01/31/2024, the employer exposed an employee to fall hazards of approximately 14-feet, 6-inches to the lower ground level, in that a employee was allowed to work from an elevated position inside a wooden makeshift platform that measured 9-feet long by 5-feet wide that was resting on the forks of a MTA 8044 rough all terrain forklift truck that was not secured to the forks or installed into the fork pockets as required by the manufacturer.

29 CFR 1926.403(b)(1):Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) New commercial construction located at 763 Highway 96, Bonaire, GA 31005. On or about 01/31/2024, the employer failed to inspect all electrical extension cords used to provide power to electric tools employees were using, in that a red and black 50-ft. electrical cord was damaged and exposing the internal insulated energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
