29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about January 16, 2024, the employer did not ensure that energy control procedures were utilized for Bagger machine 4 prior to allowing employees to clean glue under the glue tray. Employee(s) were exposed to un-expected start up or movement of the machine, resulting in exposure to caught-between hazards from machine parts that could result in amputations. b) On or about February 14, 2024, the employer did not ensure that energy control procedures were utilized for Bagger machine 3 prior to allowing employees to performing set-up. Employee(s) were exposed to un-expected start up or movement of the machine, resulting in exposure to caught-between hazards from machine parts that could result in amputations. c) On or about February 14, 2024, the employer did not ensure that energy control procedures were utilized for Bagger machine 5 prior to allowing employees to performing set-up. Employee(s) were exposed to un-expected start up or movement of the machine, resulting in exposure to caught-between hazards from machine parts that could result in amputations. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
