29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: Location - Front of the house roof. On or about 2/1/2024, an employees was exposed to injury and a fall greater than 22 feet in height to ground level from a residential roof while working where the fall protection system being used (leg/waist) was inadequate to prevent injury and/or fall.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Location - Porch roof On or about 2/1/2024, employees were not protected from falling from the roof to a lower level while working at heights greater than 6 feet. The Evelio Construction Inc was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1485398, citation number 1, item number 1 and was affirmed as a final order on 11/16/2020, with respect to a workplace located at 223 Main Street, Acushnet, MA 02743.
