29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Location: Rooftop of home On November 3, 2024, an employee was seen working approximately 30 feet from the ground without any fall protection system in place. Evelio Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(10), which was contained in OSHA inspection number 1485398, citation number 1, item number 1 and was affirmed as a final order on 11/16/2020, with respect to a workplace located at 223 Main Street, Acushnet, MA.

29 CFR 1926.502(d)(17):The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: Location - Front of the house roof. On or about 12/3/2024, employees were exposed to injuries and falls of greater than 20 feet in height to ground level from a residential roof while working where the fall protection system being used (leg/waist) was inadequate to prevent injury and/or fall.
