Instance B:   THE Crown 'double' PIT was not removed from service despite its horn not working.

Instance A:  Written LOTO procedures were not developed for several machines. Instance B:  Employees do not lock out equipment when performing some maintenance and servicing tasks as called for in the written procedures. Instance C:  Written LOTO procedures lack details that would let workers know how to successfully de-energize equipment.   Instance D:   Written LOTO procedures are not in Spanish, the primary language for most maintenance employees.    The Channel Fish Processing Co., Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number  1466770, citation number 1, item number 1,  and was affirmed as a final order on March 30, 2020, with respect to a workplace located at 200 Commerce Drive in Braintree, MA.

Instance B:   The portioner pusher plate guards safety interlock was inoperable allowing the machine to operate without the guard in place.  Instance C:   The portioner photo sensor was missing.  This allowed access to moving parts from the magazine side of the machine.  Instance D:   The portioner double redundant safety switch was missing.  Instance D:   The Curio de-heading machine's saw blade was not adequately guarded.  Instance E:   Breading machine 'breading distribution rollers' near the hoppers are not adequately guarded.  The Channel Fish Processing Co., Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number  1466770, citation number 1, item number 2,  and was affirmed as a final order on March 30, 2020, with respect to a workplace located at 200 Commerce Drive in Braintree, MA.
