Located at 281 Wooden Crossing Circle, Saint Augustine, Florida, 32084 on or about February 6, 2024 the employer exposed employees to fall hazards ranging from 9-foot to 20-foot in that supervisors did not ensure that personal fall arrest systems were being utilized.
