29 CFR  1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  On or about February 7, 2024, at establishment located at 704 Industrial Drive, Colby, Wisconsin: the employer did not provide fall protection training to an employee before being exposed to fall hazards of over 10 feet.

29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.  On or about February 7, 2024, at the establishment located at 704 Industrial Drive, Colby, Wisconsin: the employer did not develop and document energy control procedures for employees setting up, testing, troubleshooting, and servicing customer equipment located in the automated cell.

On or about February 7, 2024, at the establishment located at 704 Industrial Drive, Colby, Wisconsin: employees utilized lockout devices that did not indicate the identity of the employees while setting up, testing, troubleshooting, and servicing customer supplied equipment in the automated cell.

29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  On or about February 7, 2024, at the establishment located at 704 Industrial Drive, Colby, Wisconsin: the employer failed to protect its employees from unsafe practices while utilizing an attachment on the Hyster Model E60XN-33 forklift without written approval from the forklift manufacturer (or qualified Registered Professional Engineer) for the attachment to be used with the forklift.
