North Ridgeville OH 44039, did not ensure the side rails of one portable sixteen foot Werner brand aluminum extension ladder was extended at least three feet above the landing surface. Two employees were exposed to falls from approximately 12 feet or greater from the top of the second (roof) level to the ground below while using the ladder to access the second (roof) level to perform residential framing activities.

North Ridgeville OH 44039, did not provide a training program for each employee potentially exposed to fall hazards while using a portable ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. Two employees were exposed to falls from approximately 12 feet or greater from the top of the second (roof) level to the ground below while using a Werner brand aluminum ladder to access the second (roof) level to perform residential framing activities.
