Green St., Middletown, DE 19709 (Front Yard): On or about February 7, 2024, employees conducted framing operations while using a JGL Skytrak Model: 8042, Serial # 32202632 with an unsecured basket/platform. The operator was observed moving the JGL Skytrak Model: 8042, Serial # 32202632 while an employee was still elevated approximately 20 feet above and ungraded dirt surface.
