Employee(s) engaging in trenching activities at or about 655 North 108th Ave. Omaha, Nebraska, on or about February 9, 2024, were exposed to cave-in hazards in that the employee(s) were not instructed in the recognition of the hazards in an unprotected trench at depths greater than 10 feet.

Employee(s) engaging in trenching activities at or about 655 North 108th Ave. Omaha, Nebraska, on or about February 2, 2024, were exposed to fall hazards in that the employee(s) were not instructed in the recognition of the hazards of the ladder's side rails do not extend 3 feet above the upper landing surface used to gain access.
