Sherman Indian High School: On February 8, 2024, the employer failed to provide copies of the requested injury and illness records for the calendar years 2021, 2022, 2023 when requested both verbally and in writing.   The Bureau of Indian Education was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1960.66(b), which was contained in OSHA inspection number 1546996, citation number 2, item number 1, issued on January 19, 2021, which was affirmed as a final order on March 12, 2012 with respect to a workplace located at Tuba City Boarding School, Tuba City, Arizona 86045.
