29 CFR  1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:  (a)On or about June 20, 2024, and at times prior thereto, BUREAU OF INDIAN EDUCATION, CHEMAWA INDIAN SCHOOL, employees were exposed to occupational injury and illness when sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language was not placed on the dock leveler located on the loading dock at the rear of the school.  The CHEMAWA INDIAN SCHOOL was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.146(c)(1), which was contained in OSHA inspection number1333625, citation number 1, item number 1a and was affirmed as a final order on 02-06-2019, with respect to a workplace located at BUREAU OF INDIAN EDUCATION, CHEMAWA INDIAN SCHOOL, Salem, OR.
