The employer did not ensure the written materials are readily accessible to the employees in their work area throughout each work shift. a) Phillips 66 Company Billings Refinery, Delayed Coker Unit, Quench Water Storage Tank T-3901: On or about February 9, 2024 and at times prior, employees were exposed to fire and subsequent explosion hazards created by the carry under of hydrocarbons from settling drum (D-3912) and fines maze into the quench water storage tank (T-3901). The employer failed to identify the: hazards, consequences, safeguards, and mitigation procedures for the recirculated hydrocarbons in the quench water storage tank (T-3901).

29 CFR 1910.119(f)(1):The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process: a) Phillips 66 Company Billings Refinery, Coker Unit, Quench Water Storage Tank T-3901: On or about February 9, 2024 and at times prior, employees were exposed to fire and subsequent explosion hazards created by the carry under of hydrocarbons from settling drum (D-3912) and fines maze into the quench water storage tank (T-3901). The employer failed to implement a procedure to safeguard and monitor the accumulation of hydrocarbons in the T-3901 quench water storage tank.

29 CFR 1910.119(l)(4):Process safety information required by paragraph (d) of this section was not updated when a change covered by this paragraph resulted in a change in the process safety information: a) Phillips 66 Company Billings Refinery, Delayed Coker Unit, Oil Skimming Pump P-3948: On or about February 9, 2024 and at times prior, the employer did not update process safety information to address deficiencies and potential hazards, including, but not limited to, failure of the 4"-HD-39-377-S-A2-1 line from D-3913 to P-3948. The blinding of Pump P-3948 and associated piping from service created a dead leg in the process.
