29 CFR 1926.753(d)(2)(ii): Hooks with self closing safety latches were not used to prevent components from slipping out of the hook. a) On or about February 14, 2024, an employee was exposed to struck-by and crushed-by hazards due to the employee working under a load of steel suspended by hooks without self closing safety latches.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) On February 14, 2024, an employee performing steel erection activities on the edge of a commercial building approximately twenty-two (22) feet five (5) inches above ground was not protected from falls, thereby exposing the employee to fall hazards.
