29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On or about February 20, 2024, at an establishment located at 300 East Breed Street, in Chilton, Wisconsin; by the NRS Labeler, an extension cord, used to power a portable relocatable power tap, and a power cord for the Zebra ZE511 RFID labeler were on the floor where employees changed out stickers and the ribbon exposing the employees to slip and trip hazards.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. On or about February 20, 2024, at an establishment located at 300 East Breed Street, in Chilton, Wisconsin; by the NRS Labeler, a portable relocatable power tap (surge protector), used to power a computer, a computer screen, and a labeler, was daisy chained with an extension cord to the outlet exposing employees to shock and fire hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about February 20, 2024, at an establishment located at 300 East Breed Street, in Chilton, Wisconsin; in the PSM Fill Area, Line 3, the side panel on each side of the test tank 3 transition was not secured exposing employees to the unguarded in-running nip point.
