29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting was not provided to all employees upon initial employment, and at least annually thereafter.  a) 1104 New Holland Ave, Lancaster, PA 17601- Employees were provided with multi-purpose dry chemical extinguishers, and had not had fire extinguisher training.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  a) 1104 New Holland Avenue, Lancaster, PA 17601 - A Pelonis 1500W 3-Speed Electric Fan-Forced Space Heater (M/N:  PSH08F1AWW) was being powered in conjunction with a flexible electrical extension cord, which goes against the listing and labeling of the electric heater in that user of the heater is to plug the heater directly into a wall outlet/receptacle, on or about February 21, 2024.  b) 1104 New Holland Avenue, Lancaster, PA 17601 - A Pelonis 1500W 3-Speed Electric Fan-Forced Space Heater (M/N:  PSH08F1AWW) was being powered in conjunction with a flexible electrical extension cord, which goes against the listing and labeling of the electric heater in that user of the heater is to never use the heater with an extension cord, on or about February 21, 2024.  c) 1104 New Holland Avenue, Lancaster, PA 17601 - A Pelonis 1500W 3-Speed Electric Fan-Forced Space Heater (M/N:  PSH08F1AWW) was being used in a portable outdoor restroom (port-a-john), which goes against the listing and labeling of the electric heater was not intended for bathrooms and other similar outdoor locations, on or about February 21, 2024.  d) 1104 New Holland Avenue, Lancaster, PA 17601 - A Pelonis 1500W 3-Speed Electric Fan-Forced Space Heater (M/N:  PSH08F1AWW) was being used at a worksite in a portable outdoor restroom (port-a-john), which goes against the listing and labeling of the electric heater was intended for household use only, on or about February 21, 2024.  e) 1104 New Holland Avenue, Lancaster, PA 17601 - A Westinghouse relocatable power tap was plugged into an electrical receptacle and was being used to power equipment including, but not limited to, a second CyberPower relocatable power tap.  The second relocatable power tap was being used to power equipment including, but not limited to, cell phone chargers.  The second relocatable power tap was not listed and labeled for use in that the second relocatable power tap was series connected / daisy chained to the first relocatable power tap, on or about February 21, 2024.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

No abatement certification or documentation required.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.  a) 1104 New Holland Ave, Lancaster, PA 17601 - The flexible cord and cord cap of the Panasonic microwave did not have stain relief in that the microwave's flexible power cord and cord cap had tension on it, pulling the cord cap from the electrical receptacle, on or about February 21, 2024.  b) 1104 New Holland Ave, Lancaster, PA 17601, far left corner of the jobsite trailer - The flexible cord of a Westinghouse relocatable power tap did not have strain relief as the flexible cord was bent backwards near terminus of the cord cap, on or about February 21, 2024.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
