The employer must ensure: Fixed ladders that do not have cages or wells have: (i) A clear width of at least 15 inches (38 cm) on each side of the ladder centerline to the nearest permanent object; and  a) On or about February 12, 2024, the employer did not ensure that there was at least a 15 inch clearance from the centerline of the fixed ladder located in the Health United Building in that a permanent sprinkler supply pipe on the north side of the ladder was 11.5' inches from the ladder's centerline. As a result, employees were exposed to a 17' fall hazard while climbing the ladder with insufficient clearance.
