On or about February 22, 2024, west building (2778 N Forsyth Road) north wall: employees were exposed to fire and entrapment hazards, in that, the illuminated exit sign was detached from the wall, making it difficult to indicate the direction of travel along the exit route leading to the exit discharge for evacuation.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a. On or about February 22, 2024, East building (2737 N Forsyth Road) north wall of the paint shop: the access to (2) two electrical panel boxes were obstructed/blocked by a blue hand caddy, exposing employees to electric shock hazards. b. On or about February 22,2024, East building (2737 N Forsyth Road) south wall of the paint shop: the access to (4) four electrical panel boxes were obstructed/blocked by a black craftsman tool chest, and a flammable yellow cabinet, exposing employees to electric shock hazards. c. On or about February 22, 2024, West building (2778 N Forsyth Road) fabrication shop near paint booth: the access to (3) three electrical panel were obstructed/blocked by line x toolbox, microwave, stereo, water cooler, a flammable yellow cabinet, exposing employees to electric shock hazards.
