29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a. On or about February 02, 2024, in the west building next to the mixing room: employees were exposed to eye injuries while working with corrosive materials such as, but not limited to, Auto base Plus MM Q439 Yellow (Orange) Transparent, Instant Adhesive CA40 Clear, in that the eye wash station was empty and not capable of delivering water for at least 15 minutes.

29 CFR 1910.134(f)(2):The employer did not ensure that an employee using a tight-fitting respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece was used, and at least annually thereafter: a. On or about February 02, 2024, in the west building painting QC area: an employee was exposed to respiratory hazards during painting operations, in that the employee was required to wear a 3M 6200 Half Face respirator and was not provided with a fit test prior to initial use of the respirator.
