29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On or about February 22, 2024, and at times prior, employees were exposed to caught-in and crushing hazards from the Lock Out Tag Out program not containing machine specific energy control procedure(s), employee training, and periodic inspections.

29 CFR 1910.212(a)(3)(ii):Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about March 19, 2024, an employee was exposed to amputation hazards while operating the Cincinnati Hydraulic Sheer Cutter, Serial Number 21585, to cut sheet metal. The gap on the guard of the Cincinnati Hydraulic Sheer Cutter, Serial Number 21585, was measured to be 0.5 inches, which was big enough for the employee to get their left pinky finger into the danger zone during the operating cycle. This resulted in the employee suffering an amputation.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about February 22, 2024, and at time prior, employees were exposed to caught in hazards from the rotating chuck on an unguarded Dayton 15 inch Drill Press, Model # 5PHC2, Lot# 081200127, to drill holes in metal products.
