29 CFR  1910.212(a)(3)(ii):Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  a) On or about March 19, 2024,  an employee was exposed to amputation hazards while operating the Cincinnati Hydraulic Sheer Cutter, Serial Number 21585, to cut sheet metal. The gap on the guard of the Cincinnati Hydraulic Sheer Cutter, Serial Number 21585, was measured to be 0.5 inches, which was big enough for the employee to get their left pinky finger into the danger zone during the operating cycle. This resulted in the employee suffering an amputation.
