(a) Production Floor, Extruder 1-Line E1: On or about January 30, 2024 and times prior, employees worked on the Ettlinger High Performance Melt Filter machine. Hazardous energy control procedures were not adequately developed, documented or utilized to prevent the release of pressurized thermal energy from Polyethylene Terephthalate (PET) heated to 250-280 degrees Celsius.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) 5115 Winchester Avenue, Martinsburg, WV: On or about January 30, 2024, a worker was servicing an extrusion machine that malfunctioned causing injuries requiring in-patient hospitalization. The in-patient hospitalization was not reported to OSHA within 24-hours by the employer.
