Employees reached into the point of operation to adjust molds while the machine remained energized and no machine-specific lockout/tagout procedure was utilized.  b) Thermoforming Department: On or about September 10, 2025 and at times prior, employees were exposed to caught-in and amputation hazards while performing blade changes on Thermoforming Cutting Machine #5 (Sunwell Machines Co., LTD, Model M1370L-HTPF). Employees accessed the cutting area to change blades while the machine remained energized and no machine-specific lockout/tagout procedure was utilized.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
