29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Thermoforming Department: On or about September 10, 2025 and at times prior, employees were exposed to caught-in and amputation hazards while performing mold adjustments on Thermoforming Cutting Machine #5 (Sunwell Machines Co., LTD, Model M1370L-HTPF). Employees reached into the point of operation to adjust molds while the machine remained energized and no machine-specific lockout/tagout procedure was utilized.  b) Thermoforming Department: On or about September 10, 2025 and at times prior, employees were exposed to caught-in and amputation hazards while performing blade changes on Thermoforming Cutting Machine #5 (Sunwell Machines Co., LTD, Model M1370L-HTPF). Employees accessed the cutting area to change blades while the machine remained energized and no machine-specific lockout/tagout procedure was utilized.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR  1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) Thermoforming Department: On or about September 10, 2025 and at times prior, employees performed mold adjustments on Thermoforming Cutting Machine #5 (Sunwell Machines Co., LTD, Model M1370L-HTPF) while the machine remained energized, placing their hands in the point of operation.  Employees were not adequately trained in the recognition of hazardous energy exposing them to caught-in and amputation hazards.  b) Thermoforming Department: On or about September 10, 2025 and at times prior, employees performed blade changes on Thermoforming Cutting Machine #5 (Sunwell Machines Co., LTD, Model M1370L-HTPF) while the machine remained energized.  Employees were not adequately trained in the recognition of hazardous energy exposing them to caught-in and amputation hazards.  Abatement certification and documentation required within 10 days after abatement date. The employer did not conduct an annual inspection of the energy control procedure to ensure compliance with OSHA's requirements.  Abatement certification and documentation required within 10 days after abatement date. 29 CFR  1910.132(d)(2):The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  a) All Departments; 5115 Winchester Avenue, Suite 100, Martinsburg, WV 25405. On or about September 10, 2025 and at times prior, the employer had not developed or maintained a written certification documenting that a personal protective equipment (PPE) hazard assessment had been performed. No records identifying the specific departments evaluated, the person certifying the assessment or the date of completion were available for review during the inspection.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
