(a) 1550 Irving St. Rahway, NJ 07065 Employee(s) engaged in roofing activities at a Masonic Temple were not trained to safely operate a telescopic lift. An employee was observed loading materials onto a commercial roof without a body harness attached to the basket. This condition exposed employee(s) to a fall hazard of up to 32 feet in height. Condition(s) were noted on or about 02/23/2024.
