On or about March 13, 2024, the employer failed to conduct non-destructive testing on the Low Low Temperature Accumulator vessel to determine metal thickness loss at the facility located at 2001 Summit Ave - Hastings, Nebraska 68901 per the 2022 Process Hazard Analysis and the 2020 Compliance Audit, potentially exposing employees to hazardous ammonia gas or liquid.. The 2018 Non-Destructive Testing / Mechanical Integrity Report indicated internal metal loss on the Low Low Temperature Accumulator and recommended re-rating and additional testing.
