29 CFR 1910.23(d)(7): The employer did not ensure that grab bars extend 42 inches above the access level or landing platforms served by the ladder. On or about February 27, 2024, employees who access the roof of the Flanders Provision facility at 201 Summit Ave, Hastings, Nebraska were exposed to fall hazards in that the roof access ladder did not have side rails that extended above the access level of the roof. The siderails ended at the roof level.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 27, 2024, employees working on or near the meat auger in the Flanders Provision facility at 201 Summit Ave, Hastings, Nebraska were exposed to amputation hazards in that there was insufficient guarding to prevent access to rotating auger blades.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about February 27, 2024, employees performing energy isolation procedures on the Shanklin Heat Tunnel in the Flanders Provision facility at 201 Summit Ave, Hastings, Nebraska were using energy isolation procedures that had not been reviewed by their employer to ensure they were still current with established standards.
