The failure to conduct and certify periodic inspections of the energy control procedures created hazards to employees required to perform servicing and maintenance work on machines and equipment where the unexpected energization, startup, or release of hazardous energy could occur.

29 CFR 1910.147(c)(7)(i)(A):  Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:    a)  Jobsite, 1400 Country Ridge Road, Bedford, PA 15522:  On or about February 21, 2024, an employee required to perform preventative maintenance (PM) on the Inbound Recirculation Line which included a Intelligrated band conveyor system (Model: 470016-VB Skew WH Drive assembly, serial number: SO: 2006707) was not provided training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  The employee was injured after his right hand was caught-by  an in-running nip point on the chain and sprocket drive of the conveyor system.

The employer had no lock out training records for an employee who was injured while performing preventative maintenance on the conveyor system.

29 CFR 1910.147(d)(2):  The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:   a)  Jobsite, 1400 Country Ridge Road, Bedford, PA 15522:  On or about February 21, 2024, an Intelligrated band conveyor system (Model: 470016-VB Skew WH Drive assembly, serial number: SO: 2006707) that was part of the Inbound Recirculation Line was not turned off or shut down using the procedures established for the machine.  An employee suffered an amputation injury after being caught-by an in-running nip point.  The machine was not locked and tagged out using procedures established for the machine prior to the start of preventative maintenance work which required the removal of a chain and sprocket guard.

The top rail was not positioned 42 inches above the walking working surface (plus or minus 3 inches).  Employees working in the area were exposed to a fall hazard of approximately 15 feet to the cement floor below.

The top edge of the midrail was approximately 15.5 inches above the floor.  A gap of approximately 19.5 inches existed between the top rail and midrail.  Employees working in the area were exposed to a fall hazard of approximately 15 feet to the level below.
