At 8502 Deport Dr, Overland, MO, 63114, on February 29, 2024 and at times prior thereto employees were exposed to struck by and caught between hazards due to employees operating powered industrial vehicles prior to completing training as required.

29 CFR 1910.178(p)(1):If at any time a powered industrial truck were found to be in need of repair, defective, or in any way unsafe, the truck was not taken out of service until it has been restored to safe operating condition. At 8502 Deport Dr, Overland, MO, 63114, on February 29, 2024 and at times prior thereto employees were exposed to struck by and caught between hazards from powered industrial trucks that was not taken out of service while not in a safe operating condition. The following powered industrial vehicles were found to be in need of repair due to operational defects. Mitsubishi Model FGC25 N S/N AF82F46230, Caterpillar Model 2C5000 S/N 9046907, Caterpillar Model 2C5000 S/N AT9046909, Mitsubishi Model FGC25N S/N AF82F46101.
