29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about February 28, 2024, three employees were performing roofing operations up to approximately 10 feet from the ground to the initial working surface, without the use of fall protection; exposing the employees to fall hazards.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On or about February 28, 2024, three employees were using a ladder to climb to their work areas and the side rails of the ladder did not extend at least 3 feet above the upper landing surface; exposing employees to fall hazards.
