29 CFR 1910.178(l)(3)(ii)(C): Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace including but not limited to, fork and attachment adaption, operation, and use limitations; On or about January 27, 2024, at a located 1333 Scheuring Road, De Pere, Wisconsin in the Press Department; employees operating Yale lifts equipped with Cascade paper roll clamps were not trained in their limitations. Employees did not center the paper roll(s) in the center of the clamp attachment and pads to ensure the paper roll remained secured in the clamp while tamping gaylord containers.
