29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: In the employee break room of the Wash Rack Building, employees are exposed to unsanitary conditions in that there were spiders and cobweb by the furnace area, and the ceiling. The break room was not kept in a clean, and sanitary condition.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place: Inside Refrigerated Trailer Wash-Room - Employees engaged in material handing activities using the Nissan forklift were exposed to stuck-by hazards in that the two lever-control indicators had missing knobs to indicate the purpose and functions.
