29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about March 5, 2024, on the north and south sides of the church roof with the steeple, employees were performing roofing work activities from a roof that was more than six(6) feet to the ground below and were not in use of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about March 5, 2023, employees performing commercial roofing operations were using a portable ladder that did not extend at least 3 feet about the upper landing surface on the south side of the building with the steeple attached to it. IPR Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1673097, citation number 1, item number 1 and was affirmed as a final order on date December 8, 2023, with respect to a workplace located at 3915 Atticus Ave, Norman, OK 73072.
