29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about 3/12/2024, the employer did not protect employees who were exposed to eye hazards due to flying particles while using pneumatic nail guns. The employer did not ensure that each affected employee used appropriate eye protection while at a residential roofing worksite located at 229 Burrograss Street in Wentzville, MO.

29 CFR  1926.152(a)(1): For quantities of one gallon or less, containers other than the original container(s), approved metal safety cans, or DOT-approved containers were used for storage, use, and handling of flammable liquids:  On or about 3/12/2024: The employer did not ensure gasoline, used in the gas-powered air compressor, was transported in an approved metal safety can or DOT-approved container to ensure employees were not exposed to chemical hazards and fire hazards during construction operations at 229 Burrograss Street in Wentzville MO.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were an employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 3/12/2024, the employer did not protect employees who were engaged in residential roofing operations 6ft (1.8m) or more above lower levels while at the job site located at 229 Burrograss Street in Wentzville, MO. The employees were conducting residential roofing operations that exposed them to the hazard of falling from a height of greater than ten (10) feet.

On or about 3/12/2023, the employer did not develop, implement, and maintain at each workplace, a written communication program which at least described how the criteria in paragraphs (f) for labels and other forms of warning, (g) for safety data sheets (SDSs), and (h) for employee information and training would be met. Employees were exposed to health and chemical hazards from utilizing gas to power air compressors for the use of pneumatic nail guns at a worksite located at 229 Burrograss Street in Wentzville, Missouri.
