29 CFR 1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a) On or about March 12, 2024, at a jobsite located at 1029 Harrison St., Hartford, WI 53027, employees were operating pneumatic nail guns without eye protection while installing a new roof on an existing residential home. This exposed employees to a struck by hazard and eye impalement injury.

(a) On or about March 12, 2024, at a jobsite located at 1029 Harrison St., Hartford, WI 53027, employees conducting roofing activities on a roof with a 8:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 21 feet on the north side of the home.
