Ohio Taxidermy Supply located at 20 Straits Lane, Killbuck, OH 44637: On and/or preceding March 12, 2024, the employer did not ensure that the proper PPE was utilized throughout the in the Form, Ear, Eye, Casting, Molding, Woodshop Departments and the inventory warehouse and Z-stock area to ensure employees were protected  during taxidermic materials manufacturing/processing. The following conditions exposed employees to hazards such as but not limited to contact and inhalation hazards.

29 CFR  1910.132(f)(1):The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following, as required by 29 CFR  1910.132 (f)(1)(i) through (f)(1)(v):  a. Ohio Taxidermy Supply located at 20 Straits Lane, Killbuck, OH 44637: On and/or preceding March 12, 2024, the employer failed to provide training on personal protective equipment to employees in the Form, Ear, Eye, Casting, Molding, Woodshop Departments and the inventory warehouse and Z-stock area. The following conditions exposed employees to hazards such as but not limited to contact and inhalation hazards while working  with machinery and hazardous chemicals such as but not limited to Polyurethane Resin, Diphelemthlane Diisocyanate, Acetone, Techneglas Resin Solution (Hard Coat Resin).

Ohio Taxidermy Supply located at 20 Straits Lane, Killbuck, OH 44637: On and or preceding March 12, 2024, the employer did not provide employees operating powered industrial vehicles with training that consisted of a combination of formal instruction, practical training, and evaluation. Forklift operators did not receive formal instruction and/or evaluations of the operators' performance was not performed as required by this part.
