29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about March 13, 2024, at a jobsite located at N50 W19889 Tamarind Way, Menomonee Falls, WI 53052,  employees were operating pneumatic nail guns without eye protection while installing a roof on a new residential home. This exposed employees to a struck by hazard and eye impalement injury.  Twins Magana LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1620966, citation number 1, item number 1 and was affirmed as a final order on February 7, 2023, with respect to a workplace located at 864/872 Stonecrop Drive, Hartford, WI 53027.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about March 13, 2024, at a jobsite located at N50 W19889 Tamarind Way, Menomonee Falls, WI 53052, employees installing shingles on the back (south) side of a new residential construction home were not using fall protection. This exposed employees to a fall hazard of approximately 16 feet 11 inches from the roof eve to the ground below on the back (south) side of the home.  Twins Magana LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1620966, citation number 1, item number 2 and was affirmed as a final order on February 7, 2023, with respect to a workplace located at 864/872 Stonecrop Drive, Hartford, WI 53027.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) On or about March 13, 2024, at a jobsite located at N50 W19889 Tamarind Way, Menomonee Falls, WI 53052,  employees accessing the roof to conduct roofing work from a ladder which did not extend at least three feet above the roof.  Twins Magana LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1620966, citation number 1, item number 4 and was affirmed as a final order on February 7,2023, with respect to a workplace located at 864/872 Stonecrop Drive, Hartford, WI 53027.

(a) On or about March 13, 2024, at a jobsite located at N50 W19889 Tamarind Way, Menomonee Falls, WI 53052, the employer did not verify fall protection training by preparing a written certification record for employees tasked with installing shingles on a new residential home.
