29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At jobsite located at 11414 E Fairfax St, Wichita, KS 67226, employees engaged in residential roofing activities were exposed to fall hazards of approximately 12 and 18 feet from the working surface to the ground in that fall protection was not utilized. Samuel Baca was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1508840, citation number 1, item number 3, and was affirmed as a final order on 03/18/2021, with respect to a workplace located at 7712 Turquoise Trail, Bel Aire, KS 67220.

29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the jobsite located at 11414 E Fairfax St, Wichita, KS 67226, employees engaged in residential roofing activities were exposed to falls from approximately 12 feet, in that the Gorilla aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof. Samuel Baca was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1508840, citation number 1, item number 5, group letter a, and was affirmed as a final order on 03/18/2021, with respect to a workplace located at 7712 Turquoise Trail, Bel Aire, KS 67220.
