29 CFR  1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  a.  On or about March 15, 2024, an employee required to conduct welding and grinding operations was exposed to burn hazards when not provided or required to use adequate body protection to protect from hot sparks and/or metal.  The employee was exposed to the burn hazard when the welding jacket worn during welding operation(s) was visibly damaged and ripped.  b. On or about March 15, 2024, an employee required to conduct welding and grinding operations was exposed to burn hazards when the employer failed to provide the employee(s) with appropriate body protection, including but not limited to a welding jacket.

29 CFR  1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:  a.  On or�about March 15, 2024,�in the�facility, employees�were not�trained to�know at�least the�following:    1.  What PPE is necessary.   2.  When PPE is necessary.  3.  How to properly don, doff, adjust and wear the PPE. 4. The limitations of PPE.  5.  The proper care, maintenance useful life and disposal of the PPE.    Employee exposure included, but was not limited to, burn hazards from welding and cutting operations.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a.  On or about March 15, 2024, in the facility, all employees had not been trained with effective information on hazardous chemicals in their work areas such as, but not limited to, nitrogen and carbon monoxide.
