On or about March 15, 2024, at the site, the employees were exposed to a fall hazard of 10 feet on a 5:12 sloped roof, in that the use of fall protection was not enforced while the employees were repairing the roof shingle.

On or about March 15, 2024, at the site, employees were using an extension ladder to access the roof, and the side rails were approximately 30 inches past the roof landing (2 rungs and the extension), exposing employees to fall hazards of approximately 10 feet.
